For most of us, the main problem with failure will be over inflation, which will usually occur on shore. On the water failure is usually rare. Punctures on the water can come from a variety of things, TD plunging a knife into his tube ( seen him do it ), kitty catfish spining it ( done that one my self) are the most common. Usually with both of these, the leak will be fairly slow and you should be able to kick to shore without to much trouble. If you are wearing a wader belt ( duh !!) and a LIFE JACKET !! you should be able to get off the water in most situations without to much difficulty. If you are using that thing between your ears for more than a hat rack and using common sense, you should not have to much difficulty with it.
